Many facilities need “Proof of Insurance” for groups and their meetings. Sometimes they may accept this form for events. If the facility needs to be named as “Additionally Insured”, please fill out the insurance request form
Please fill out the form completely. Only “organized” sports (baseball tournaments etc..) need to be checked. Playing games like horseshoes, swimming, etc. at facilities that are open to the public do not need to be listed. Bounce houses, Dunk tanks and other commercial entertainment must include a certificate from the vendor naming “California Mid-State Region of Narcotics Anonymous “as “ Additionally Insured”. This must be submitted with the request form! When filling out the insurance request form please send from a valid email address. The “Insurance Certificate” will be sent back to this email.
